Alahai Migrain...




Salam,

Beberapa hari yang lepas, saya telah membuat temujanji untuk melakukan ujian HSG di Hospital Tawakal. Temujanji tersebut adalah hari Isnin minggu depan. Masalahnya, sejak 1st day saya mendaftar di LPPKN iaitu pada 6hb April mggu lepas, kepala saya tak berhenti-henti berdenyut. Mula-mula ingatkan pening biasa. Lama-kelamaan ia menjadi makin teruk. Saya rasa mungkin saya migrain, atau mungkin jugak sebab tekanan darah saya tinggi. Masa check blood pressure kat LPPKN hari tu, nurse tu terkejut jugak bila bacaannya tinggi, around 140/120... 3x jugak nurse tu ambik:-) tapi bacaan still remain lebih kurang camtu lah. Nurse tanya saya tak sihat ke? Masa tu mmg saya rasa tak berapa sihat... period dtg byk membuak-buak (maklumlah sebelum nie period tak menentu, actually dh 7 bulan dia tak dtg dgn banyaknya... selepas makan ubat turun darah, baru keluar banyak), sakit perut, sakit kepala dan selesema...nak kata apa lagi..mmg tak sihat la tuh...:-)

Asal2 memang saya takde high blood pressure, cuma ada darah gemuruh je. Kalau saya tertekan atau terlalu pikirkan sesuatu masalah, darah tinggi akan naik. Kalau saya pi klinik/hospital untuk jumpe doktor pun jantung saya akan berdebar2 pelik. Ntahla, mgkn fobia dengan doktor masa kecik2 dulu kot... saya pun dah tak ingat...:-)
Sebelum ni kalau doktor ambik BP, mesti dia akan kata BP saya kat tahap 'borderline'. Kalau lepasi sempadan tu, kira jadi high BP la...:-) tapi yg peliknya kalau doktor yg saya jumpe tu peramah dan sempat sembang2 dulu sebelum dia amik tekanan darah, BP saya jadi normal dalam 120/80 gitu...:-) Peliklah darah gemuruh saya nie... susah nak kawal dan memang2 menyusahkan..:-(... (kalu sesiapa ada petua kurangkan darah gemuruh, share la yer.. saya pun nak tau...)

Ok berbalik kepada sakit kepala saya tadi... hari nie sudah hari keenam sakit kepala.. dia dtg ikut time dia sendiri, bangun tido pagi2 tak rase sakit lagi, tapi dalam kul 12 lebih, dahi kanan mula start berdenyut2... kul 2-5 ptg, kena berbaring sbb sakit sangat2... kadang2 kalau tak tahan kena makan panadol 2 bj untuk ease the pain...dalam kul 6 ke atas, sakit dan denyutnya berkurangan hingga ke malam hari, tapi still sakit la sebelah kanan kepala, cuma boleh tahanla.... macam tu lah sakit kepala yg saya alami setakat 6 hari nie..

Hubby kata sakit kepala mgkn sebab saya terlalu pikir pasal ujian hsg tu... mungkin jugak sebab siang malam saya asyik duk pikir pasal doktor akan masukkan alat dalam *tut* saya tuh.... so hubby suruh pikir baik2 la sama ada nak buat sekarang ke atau nak tunda bulan depan jer... dia sokong jer apa2 keputusannya sbb dia kata yang kena tanggung sakit nye sapa lagi kalu bukan bini dia...:-) so 2 hari lepas saya ambik keputusan untuk tangguh dulu Hsg hingga ke bulan depan sbb asyik duk sakit kepala jer... hubby kata jgn fikir lagi la pasal hsg tu... so saya ikut nasihatnya, saya tak pikir langsung pasal hal tu..tapi kepala saya still sakit berdenyut2...:-(

Pagi tadi saya baru call Hospital Tawakal utk cancel saya punye apointment hsg..takde masalah, cuma next period nnt saya kena call untuk set apointment baru... ptg tadi saya ada 'tanya' encik google ttg masalah sakit kepala saya nie di mana ia hanya sakit dan berdenyut2 di bahagian dahi kanan dan sekeliling mata kanan... dan berdasarkan simptom2 tu, ia mgkn berpunca daripada masalah sinus...hmmm... iye ke? ada kemungkinan jugak sebab selesema saya tak baik lagi, dan lagi satu, sinus berkait rapat dengan resdung, dan mmg saya ada penyakit resdung... hmmm.... entahlah apa punca sebenarnya sakit kepalaku ini.... kalu gi klinik, mmg sah2 doktor akan kata tekanan darah tinggi, dan mungkin akan prescribe ubat darah tinggi... itu yang saya taknak...

Satu lagi perkara yg saya duk pikir skang nie, adakah sakit kepala ini akan menganggu ovulasi saya untuk bulan nie? dari apa yg saya baca, sakit kepala atau apa2sakit MUNGKIN akan menganggu keseimbangan hormon (dahlah hormon saya sudah lama tak seimbang:-(..) huhuh... macam2 masalah...tapi saya sentiasa berdoa agar Tuhan tetap beri saya peluang untuk mempunyai zuriat...aminnn......

Coping with Fertility Test Anxiety


Fertility test anxiety is a normal, albeit uncomfortable, part of infertility. Before a fertility test, you may be worried about what the test will be like, and after the test, you may be obsessing about the results.

Unfortunately, I don’t have a magic way to totally get rid of fertility test anxiety. What I can share are some tips that may help lessen some of the anxiety and provide you with some practical ways to cope.

Coping with Anxiety Before a Fertility Test
Ask questions from your doctor about what you can expect. While you’re most likely familiar with blood work, other testing, such as ultrasound, HSG, or diagnostic laparoscopy, may be new experiences. The more you know about a test, the lower your anxiety will be.

Questions you may want to ask include:

Why are you recommending this fertility test?
What can I expect during the test?
What are the risks involved?
Will the test hurt?
Can my husband/friend/partner accompany me during the test?
How long after the test will I receive the results?
Will my insurance cover the test? If not, how much will the fertility test cost?
When asking questions, make sure to ask about preparation for the test and how you can expect to feel afterward. For example, some doctors will suggest taking some ibuprofen before an HSG to help with cramping. After a diagnostic laparoscopy, you will need someone to drive you home and you may need to take a couple days off work to recover from the surgery.

Besides asking your doctor questions, you can also ask friends, either online or from “real life,” about their experiences. Fertility forums are a place where you can find people who have gone through testing before, and who might have helpful advice and words of encouragement.

Something that helped me a great deal with fertility test anxiety was a guided imagery audio program from Belleruth Naparstek called Help with Fertility. I listened to the relaxation track many times before having a hysteroscopy, including in the car on the way to the exam!

Coping with Anxiety During the Fertility Test
A blood test may take only a few minutes, but if needles make you nervous, a few minutes may feel like hours. Other fertility tests can take up to half an hour or more, and when you’re nervous, half an hour is an eternity.

Here are some things you can do to cope with anxiety during a fertility test:

If permitted, bring along a friend or your partner. Be sure to check beforehand if you’re allowed to have someone with you, so you’re not surprised at the last minute that you’ll need to be by yourself for the exam. Even if your partner can’t come into the exam room with you, knowing they are on the other side of the door can help.

Don’t forget to breathe. People have a natural tendency to hold their breath or breathe very shallow breaths when they are nervous, and this can increase your anxiety. Make a conscious effort to take slow, deep breaths whenever you can. (Of course, if the doctor tells you to hold your breath while they take an x-ray, then hold your breath! But between those moments, you can take slow, deep breaths.)

Let the examiner know that you’re nervous. Trust me, you’re not the first to be nervous during an uncomfortable fertility test, and letting the examiner know you’re nervous allows them to offer their sympathies and possibly offer comforting words. Plus, I’ve found that trying to hide that I’m anxious just adds another layer of anxiety -- revealing your nervousness helps.

Coping with Anxiety Regarding Fertility Test Results
Sometimes, you’ll get the results of a fertility test right away. Other times, you’ll need to wait, which is never easy. Staying busy, trying to live life despite wanting to live by the phone, and talking or blogging about your anxieties while waiting can all help.

Though very tempting, it’s probably best not to search the Internet for all the horrible things that might be found wrong from a test you’ve taken. Instead of trying to predict the worst, just hang in there and wait to hear what your test results actually showed.

Ideally, you asked your doctor when to expect to hear about test results. If that time period passes, don’t be shy about calling your doctor’s office and asking if the results are in.

When you get the test results, be sure to ask your doctor what they mean, what your options are, and what the next step is. If the test results show that everything is okay, ask whether you’ll need to have further testing.

Something to keep in mind is that while the majority of infertility cases can be linked to a cause, up to 10% of couples never find out what’s behind their infertility (known as unexplained infertility).

Hearing that your doctor doesn’t know why you can’t get pregnant can be frustrating. However, not having an official diagnosis or cause does not mean you won’t be treated. Unexplained infertility may be treated with lifestyle changes, fertility drugs, IUI, or IVF. Treatment can be a bit hit or miss, though, since the doctor does not know exactly why you can’t get pregnant.

Question: Will I feel pain during an HSG?
Many women wonder if the hysterosalpingogram (HSG) test will cause pain. An HSG is a special kind of x-ray that involves giving an iodine dye via the cervix in to the uterus and fallopian tubes and then taking x-ray pictures.

Does an HSG hurt?

Answer:
You may or may not experience cramping during an HSG. Some women report mild-to-moderate cramping, some don't feel much of anything and very few report severe cramping. Personally, I didn't experience more than mild cramps, and it wasn't nearly as bad as I imagined it would be.

The insertion of the device used to inject the dye in to the cervix may remind you of how a pap smear feels. When the dye is injected, you may feel a strange warming sensation. If one of the fallopian tubes is blocked, you may feel pain from the pressure of the dye against the blockage.

If you tend to feel pain during regular pelvic exams, you may be more likely to experience cramps.

Ibuprofen, taken an hour before the test, can help. When cramping does occur, it usually lasts a short five minutes, but some women have mild-to-moderate cramping for several hours after the procedure. Over-the-counter pain medication can help with cramping after the test.

Anxiety and fear about the test can increase your perception of pain. The test can be very awkward, with this big x-ray machine hovering over you while you're lying on your back, legs apart, with the speculum inside. They may ask you to roll over to your side for an x-ray or two, and you have to do it with the speculum still between your legs. It's normal to feel nervous, and relaxed breathing through the procedure can help. A nurse offered to hold my hand, and I took her up on the offer.

While mild cramps are normal, if the pain seems to be increasing after the test or you develop a fever, be sure to contact your doctor. There is a rare risk of infection following an HSG, and increasing pain may hint to a brewing infection.

All About HSG (Hysterosalpingogram)


What is an HSG (Hysterosalpingogram)?
A hysterosalpingogram (HSG) is a special kind of x-ray. Hysterosalpingogram is pronounced /he-ster-o-sal-ping-o-gram/, but luckily, almost no one calls it by the full name. They just say HSG.

An HSG is an outpatient procedure, which takes no longer than a half hour. It involves placing an iodine-based dye through the cervix and taking x-rays to help evaluate the shape of the uterus and whether or not the fallopian tubes are open or blocked.

When is an HSG Ordered?
An HSG may be ordered as part of an infertility work-up. It's also commonly ordered if a woman has experienced two or more miscarriages, since abnormal uterine shape can cause recurrent pregnancy loss.

Why is the HSG Test Important?
The HSG helps the doctor check out two important factors:

Whether or not the fallopian tubes are blocked or open.

If the fallopian tubes are blocked, a woman will not be able to get pregnant, because the egg can't meet the sperm. You can read more about diagnosis, causes, and treatment for blocked fallopian tubes here:

Whether or not the shape of the uterus is normal.

In 10% to 15% of women with recurrent pregnancy loss, an abnormally shaped uterus is to blame. Some uterine abnormalities can be treated with surgery. You can read more about the connection between uterine shape and miscarriage here:

How Do You Prepare for an HSG?
An HSG is usually preformed after your period, but before ovulation. This way it won't interfere with a pregnancy. This commonly falls out between day 10 and 12 of your menstrual cycle. Talk to your fertility clinic about scheduling the HSG, which might be tricky if you have irregular periods.

The HSG is performed while you are awake and does not involve general anesthesia, so you won't need to fast the day or night before. Your doctor may suggest taking a pain killer like ibuprofen an hour before your HSG is scheduled. This can help with the discomfort of the test. Also, some doctors prescribe antibiotics, to guard against infection.

How is an HSG Done?
For the test, you'll lie on an examination table, usually one with stir-ups like the ones found in a gynecologist's office. The doctor will perform a quick pelvic exam, and place a speculum inside your vagina. Then, the doctor will place a device called a cannula into your cervix.

Next, the doctor will inject an iodine-based dye through the cannula. After the dye has been injected, the doctor will lower an x-ray device over your pelvic and lower abdominal area. For each x-ray picture, you'll be asked to hold your breath for a moment or two. You may need to change your position, and lie on your side, for some of the x-rays.

In some women, the test may cause some cramping.

Does an HSG Hurt?

Once the doctor has decided that the pictures are satisfactory, the x-ray machine will be lifted up, the speculum removed, and you're free to go home.




What are the Risks of an HSG?
Generally, an HSG is considered to be a safe procedure. Still, there are potential risks.

In less than 1% of cases, infection may occur. This is more common if there has been previous infection of the fallopian tubes. If you experience fever or increasing pain after the test, you should call your doctor.

Other potential risks include fainting during or after the test and iodine allergy (rare). If you are allergic to iodine or shellfish, tell your doctor before the test. If you have any itching or swelling after the test, talk to your doctor.

Is the Radiation from an HSG Safe?
An HSG involves a very low amount of radiation. The low amount does not seem to causes any unwanted effects, even if you get pregnant later that cycle.

However, an HSG should not be done during pregnancy.

How Will I Feel After the Test?
You may experience mild cramps and light spotting. Over-the-counter pain relievers should help with cramps. (See this article on HSG and pain for more information.)

Generally, you'll be able to resume normal activity after the test. Some doctors may tell you to refrain from sexual intercourse for a few days after the test.

Are You More Likely To Get Pregnant After an HSG?
What Do the Results Mean?
If the x-ray shows a normal uterine shape, and the injected dye spills freely out from the ends of the fallopian tube, then the test results are considered normal. Tubal factor infertility or abnormal uterine shape is probably not the cause for your infertility.

However, if further testing does not reveal a cause for the infertility or recurrent pregnancy loss, your doctor may order a hysteroscopy to double check the findings of the HSG, specifically to recheck for problems with the uterus. (Hysteroscopy involves placing a thin, telescope like camera through the cervix, to look at the inside of the uterus.) One small study found a 35% incident of false negatives with an HSG. In other words, the HSG showed a normal uterine shape, but a hysteroscopy showed abnormalities.

Also, an HSG is not able to diagnosis or rule out endometriosis.

What Happens If Results Are Abnormal?
If the dye shows an abnormally shaped uterus, or if the dye does not flow freely from the fallopian tubes, there may be a problem. It's important to know that 15% of women have a "false positive," where the dye doesn't get past the uterus and into the tubes. The blockage appears to be right where the fallopian tube and uterus meet. If this happens, the doctor may repeat the test another time, or order a different test to confirm.

An HSG can show that the tubes are blocked, but it can't explain why. Your doctor may order further testing, including exploratory laparoscopy or a hysteroscopy, to investigate and possibly correct the problem.

Pengalaman pertama ke LPPKN


Salam,

Sudah lama saya tak update blog rupanyer...:-) Sebenarnya, sebelum nie takde hal yang menarik berlaku... hinggalah ke hari nie..

Hari nie merupakan kali pertama saya dan suami mendaftarkan diri di klinik fertiliti LPPKN. Sebelum nie saya hanya duk baca pengalaman orang lain je.. :-) slps 6 tahun belum dikurniakan zuriat, kami membuat kptsn untuk try LPPKN.. 2 tahun ada buat treatment dengan Dr Hamid Arshat, tapi sekejap jer... lepas dia cadangkan ovarian drilling, saya pun naik takott... takut dgn prosedurnya dan takut dengan KOSnya yang tinggi...:-) and then lepas tu mulalah saya start try pelbagai herba untuk try lawaskan period, but to no avail... takde yg berkesan...

Sblm saya ceritakan pglmn saya pi LPPKN Jalan Raja Laut, izinkan saya maklumkan kepada para pembaca semua yang saya pengidap PCOs atau Polycyctic Ovarian Syndrome. Dr Hamid telah sahkan perkara tu bila dia tgk ovari saya bengkak masa buat transvaginal scan (scan rahim ikut bawah). Orang yang ada PCOs nie biasanya period tak teratur, tumbuh bulu-bulu halus di muka dan anggota badan yang lain, kadar testosteron tinggi, mudah naik berat badan, kulit berwarna kehitaman kat area spt siku, lutut, leher dsb. Ok, tamat citer pasal PCOS... kalau nak maklumat lanjut, boleh mintak bantuan Mr.Google, yer..:-)

Ok, sambung balik citer LPPKN. So hari nie kami bertolak pd pukul 7 pagi dari Bangi. Nak pi KL tu jauh, lagi-lagilah jem tengah jalan... 1 jam setengah baru sampai kat LPPKN Jln Raja Laut... sebelum sampai best pulak saya tengok bangunan SOGO kat tepi jalan tuh... heheh... dah lama pulak tak pi window shopping rasenyer... ok ok back to the story. Kami sampai tepat jam 8.30 dan parking kat Hotel parking kerana hotel tu duk betul2 sebelah LPPKN...

Lepas tu kami pi tingkat 6, dan mendaftarkan diri sebagai pesakit baru. Nampaknya orang tak berapa ramai, tak sampai 50 orang rasanya.. tapi nombor giliran yang kami dapat 503...heheh.. so masa 1st time registration nie, yg paling utama, suami kena buat ujian sperm...kalu kptsn ujian sperm nie OK, barulah bleh continue dengan isteri pulak...

So, dalam kul 9 lebih, suami diminta naik ke tingkat 7 untuk settlekan bisnesnya...:-D ... sementara saya diinterviu dengan seorang mesi untuk dptkan bbrp maklumat yang akan dimasukkan ke dalam komputernya. Cth soalan: brapa kali smggu bersama dengan suami, sakit tak masa *tut*, pernah main dgn orang lain tak sebelum kahwin (pelik soalan nie huhuh), dan sejarah penyakit ibu bapa, pernah buat treatment tak sblm nie etc....

Lepas selesai diinterviu, saya tunggu suami pulak. Dia selesai dalam lebih kurang pukul 10 lebih... and then tunggu lagi... dlm kul 11 lebih, suami pulak yg kena intebiu.. kesian kat hubby.. masa tu dia dah komplen lapar...laparr... ye lah tak makan ape ape semenjak pagi kan... lebih2 lagi suami saya nie jenis yg makan ikut waktu... kalu terlambat makan, memang akan lapar giler la... saya pulak jenis yg makan tak ikut waktu heheh...selagi boleh tahan, tahan lagi...:-)

Selepas suami selesai intebiu, saya pun suruh dia pi makan kat kantin bawah tue..tingkat 2... saya malas nak ikut sebab time tu period blood tgh kluar banyak sgt, dah la tak sedap badan dan hidung pulak tersumbat sbb selesema...ishh macam2 la... lupe plak nak bagitau.. kalau nak pi LPPKN, 1st day period kenal call buat appointment dgn dorang... maybe dorang akan cadangkan pi klinik masa 2nd, 3rd, 4th atau 5th day period... sebab darah hanya boleh diambil masa period hari 2 - 5 SAHAJA...

Ok, so kami pun dengan sabar menunggu result sperm test. Berjam-jam gak la kami kena tunggu nye.. mulut saya asyik duk ngomel kat hubby napelah kena tunggu lama sgt... (macamlah hubby tau jawapannya:-D).. saya pulak dah tak selesa sgt sbb period dtg banyak.perut pun sakit pulak tu.. tup tup tgk jam dah kul 1.. dan ada satu lagi couple selain kami... seperti yg saya jangka, nama kami dipanggil last sekali... hahah..

Mesi tu bagitau yg kptsn sperm suami OK... Alhamdullillah...so skrg bini plak yg jadi target nya... awal2 lagi saya dah tau yg masalahnya pada saya...:-) ok, mesi kata nnt kena buat appointment dengan hospital Tawakal (atau hospital lain) untuk buat ujian X-ray HSG... yg nie saya TAKOT dengornya...saya dah baca 1001 pglmn tentang ujian nie...rata2 orang yang telah alaminya kata SAKIT... tapi ada jugak yg kata tak brape sakit.. rasanya depends kepada pain-tolerant badan masing2 la kan..? tapi saya nie mmg confirm PENAKUT 100%... ada darah gemuruh plak tu..bayangkan setiap kali masuk hospital atau klinik untuk jumpe doktor, blood pressure akan naik... kalu nak lawat pesakit, BP tak naik pulak...heheh..

Oklah so nnt saya kena buat ujian HSG dalam hari Khamis atau Jumaat mggu depan...hari isnin depan baru nak call Klinik Tawakal nak set apointment.. ok habis je mesi citer pasal ujian HSG tu, dia cakap nak ambik darah pulak... kerana tubuh badan saya yang montel2 nie, lepas 3 kali try out, baru jumpe urat... tu pun 3 orang mesi yang godek-godek tangan saya... mrk cakap urat saya halus sgt2... sbb tu susah nak nampak...nasib baik semua mesi tu peramah... so taklah takut sgt nk kena cucuk nie, walaupun rasalah sakit sikit2...:-)

Ok settle bab cucuk mencucuk, kena buat bayaran plak la... caj untuk hari nie RM205... registration, ujian sperm dan ujian darah... kami dah ready dgn duit nya kerana awal2 lagi mereka dah bagitau suruh bawak byk tu... nnt untuk ujian HSG plak dalam RM180 kalu buat kat Hospital Tawakal...

Pukul 1.30 kami stat turun lif untuk pi kat parking kereta... sebelum tu hubby gi bayar parking autopay kat dalam hotel... nak tau brapa kena bayar...? Rm20 beb... hubby punyalah duk ngomel sbb 1st time kena paking mahal camtu... Rm4 per hour... masuk 8.30 – 1.30 ...so u do the math...

So nie lah pglmn kami pi LPPKN kali pertama...mggu depan HSG, and then 26hb April nie kena pi sekali lagi LPPKN untuk ambik darah (period day-21)...
Arghh... (sekali lagi) takutnya nak buat HSG... walaupun faedahnya byk pada saya... ntah macammana nanti... mungkin terpengsan kat situ kot...? sama2 lah kita menanti pglmn Bahagian ke 2 nanti...huhuh...
